Proof of travel will be required; you must provide our office with orders or an approved Memo in Lieu of Orders. If you are a member of FMS and attempting to use the FMS Waiver signed by Brig General Safranek you need to email afmc.fms.passportandvisa@us.af.mil to receive a control number to be approved to use the FMS Waiver with our office.

You must already have an official passport in hand with 6 months of validity left to apply for a VISA. Proof of travel will be required; you must provide our office with orders.
Waivers or Memo in Lieu of Orders will not work to apply for a VISA.

If you have been notified by our office that your passport/VISA has arrived at our facility, please book an appointment for pick-up. Ensure you bring a valid photo ID for pick-up. No further documentation is required.
If you are out-processing due to separation or retirement and have an official passport in your possession, you must surrender your official passport. Regardless of official passport status you must create yourself an appointment to be signed off by our office.
